HANDICAPPED CHILDREN

    In the case of a handicapped child whose group 
coverage is continued beyond the age of 19, proof of 
incapacity or dependency must be furnished at the 
request and expense of the insurer.  (Does not apply to 
transfer case situations where coverage is provided 
under the New York Life plan to a handicapped child who 
is over the limiting age and who was being continued 
under the prior plan's handicapped child continuance.)

dependents' life insurance as well as medical care.

also available if handicap is due to mental illness or 
developmental disability.

HANDICAPPED CHILDREN

    For all cases, eligible dependents include any 
unmarried children who are dependent upon the insured 
because of mental retardation or physical handicap and 
for whom proof of such retardation or handicap is 
provided within 31 days after the plan's effective 
date.  Such children are eligible for health insurance, 
but not for any dependents' life insurance.

REQUIRED OUTPATIENT SERVICES

    Benefits for covered laboratory tests, x-rays, 
chemotherapy, radiation treatment and renal dialysis 
performed on an outpatient basis are the same as if 
performed on an inpatient basis.
